Te Taketake 2025 Application Pack

The application form is an editable PDF you can type straight into it or print it out and fill it in by hand.

1. Don’t forget to answer the essay question (page 7, question 23).  
2. Also remember to sign it either digitally or by hand (page 10).

Provide certified copies of two forms of ID.

1. One form of ID must be photographic.
2. Please see the ID Verification Notes document for accepted forms of ID and who can verify them.
3. Ensure the person verifying your ID makes a statement to the effect of "I have sighted the original document and the copy provided is a true copy of the original" followed by their name, title, signature and date.
4. If you would like to bring original ID into our office in Dunedin for sighting please call me on 027 214 0347 to arrange a time to come to 403 High Street, Dunedin with your ID and I can sight and copy it for you.

Please include a copy of your current CV and contact details for two referees.

Complete Section 2 (pages 3 and 4) of the Police Vetting Form. We use the ID you have provided above for this as well.

We need a letter on letterhead from your employer or clinical placement confirming that you can complete your 400 hours (200 per year) of clinical practice with them.

1. Included with this pack is a Placement Requirements Document - Either email or print a hard copy of this document and give it to your employer/placement.
2. Please ask them to confirm they have received and understand it in your confirmation letter.

We ask that you have been in recovery and free of offending against others for a minimum of 2 years before applying. Please write a short declaration / attestation about this if it is applicable to you.

To include copies of the following (if applicable).

1. Any professional membership certificates (for example DAPAANZ).
2. Academic Transcript.
3. NZQA Record of Achievement - as Te Taketake is a Level 7 Diploma we do require students to have studied at Level 4 or higher.
